How to book
1) Please read this health questionnaire BEFORE purchasing this offer. If you answer YES' to any of the questions please call FST Academy on 0161 277 9848 to discuss whether you are suitable for the course. If you answer 'NO' to all the questions, proceed with the booking. Please print out and complete the questionnaire and take it to your first session.
2) Click the Buy Now button below to pay a deposit of £35. You will be directed to PayPal where you can use your credit or debit card to pay your deposit - you don’t need an account. Remember to print out your receipt.

3) Call FST Academy on 0161 277 9848 to book your first session. Please pay the remainder of £65 at your first session (by cheque or cash). Please take your PayPal receipt and completed health questionnaire to the first session.
